1A. Submit Records

Medical Records

Submit to us previous relevant medical records including pathology results and office note documentation from other healthcare providers, including urologists and oncologists.

Pathology and Lab Results

Submit all prior pathology results and prior PSA lab results along with most recent lab work, blood tests, and urinalysis results to help us understand your health status.

MRI/Imaging Reports and Images

Submit all prior MRI, CT scan, PET-CT scan and other prostate imaging reports. Please contact your radiology facility and arrange for a CD to be mailed to us or online portal access containing prior MRI, CT and PET-CT scan images.

Insurance Information

Bring your insurance card and a valid photo ID to help us process your visit efficiently.
