Pathology and Lab Results

Submit all prior pathology results and prior PSA lab results along with most recent lab work, blood tests, and urinalysis results to help us understand your health status.

MRI/Imaging Reports and Images

Submit all prior MRI, CT scan, PET-CT scan and other prostate imaging reports. Please contact your radiology facility and arrange for a CD to be mailed to us or online portal access containing prior MRI, CT and PET-CT scan images.
